Here is a quick list of the main components of a Commercial Eviction Notice for Arizona:
The Date the rent amount was due, and which property this refers to
The amount required to pay including late fees, and how many days they get (typically 5 in Arizona)
The day the property must be surrendered if they do not pay in full
The state which the laws are enforced (Arizona)
Then signature areas for the person who delivered it, and if they accepted or whom they gave it to
The notice should be signed by the tenant that they acknowledge what’s happening
